Profil Penderita Diabetes Melitus Tipe 2 dengan Hipoglikemia di Rumah Sakit Rujukan Tersier di Bali Indonesia

Abstract

Introduction: Hypoglycemia is a complication that is often found in Type 2 Diabetes Mellitus (T2DM) patients. Hypoglycemia can cause morbidity and mortality in some cases. Based on these facts, studies on hypoglycemia in T2DM patients need to be carried out to improve the outcome of T2DM patients who experience hypoglycemia. This study aims to explain the profile of T2DM patients who experienced hypoglycemia in the period October 2020 to September 2021 at a Tertiary Referral Hospital in Bali.

Method: This research uses a cross-sectional design using total sampling in collecting data from October 2020 until September 2021. Data is presented in the form of descriptive data from our electronic medical record.

Results: From 148 T2DM patients who were hospitalized from October 2020 to September 2021, 90 patients (60.8%) were male, 83 patients aged <60 years, and 83 patients aged <60 years (56.1%), and 85 patients (57.4%) used insulin. Of the 148 T2DM patients, 24 patients (16.2%) experienced complications of hypoglycemia. The incidence of hypoglycemia was dominated by male patients, 13 patients (54.5%), with the most common age ≥ 60 years, 17 patients (70.8%). The incidence of hypoglycemia in T2DM patients occurred more frequently in patients using oral antihyperglycemic drugs, 16 patients (66.7%). Patients with eGFR <15 ml/minute most often experienced hypoglycemia, namely 7 patients (29.2%). Patients who experienced hypoglycemia were dominated by patients with HbA1C > 6.5%, namely 9 patients (37.5%)

Conclusion: Hypoglycemia patients were more likely to be elderly, male, and using oral anti-hyperglycemia drugs, with HbA1C >6.5% and eGFR <15 ml/minute.

 

Latar belakang: Hipoglikemia adalah salah satu komplikasi yang sering terjadi pada pasien Diabetes Melitus Tipe 2 (DMT2). Hipoglikemia dapat menyebabkan morbitas dan mortalitas yang signifikan. Penelitian ini bertujuan untuk menjelaskan profil pasien DMT2 yang mengalami hipoglikemia pada periode Oktober 2020 sampai dengan September 2021 di Rumah Sakit Rujukan Tersier di Bali.

Metode: Penelitian ini menggunakan desain potong lintang dengan menggunakan total sampling dalam mengumpulkan data pasien diabetes melitus secara retrospektif dari bulan Oktober 2020 sampai September 2021 melalui rekam medis. Data disajikan dalam bentuk data deskriptif.

Hasil: Dari 148 pasien DMT2 yang dirawat inap dari periode Oktober 2020 hingga September 2021 didominasi oleh pasien berjenis kelamin laki-laki sebanyak 90 pasien (60,8%), pasien berusia <60 tahun sebanyak 83 pasien (56,1%), dan menggunakan insulin sebanyak 85 pasien (57,4%). Dari 148 pasien DMT2 tersebut terdapat 24 pasien (16,2%) yang mengalami komplikasi hipoglikemia. Kejadian hipoglikemia ini didominasi oleh pasien berjenis kelamin laku-laki sebanyak 13 pasien (54,5%), dengan usia tersering ≥ 60 tahun sebanyak 17 pasien (70,8%). Kejadian hipoglikemia pada pasien DMT2 ini lebih banyak terjadi pada pasien yang menggunakan obat antihiperglikemik oral 16 pasien (66,7%). Pasien dengan laju filtrasi glomerulus (LFG) <15 ml/menit paling banyak mengalami hipoglikemia, yaitu sebanyak 7 pasien (29,2%). Pasien yang mengalami hipoglikemia didominasi oleh pasien dengan HbA1C >6,5% yaitu sebanyak 9 pasien (37,5%)

Simpulan: Pasien hipoglikemia lebih banyak terdapat pada pasien tua, berjenis kelamin laki-laki, dan menggunakan obat antihiperglikemia oral, dengan HbA1C >6,5% dan LFG <15 ml/menit.
